Terms and conditions for Super Fit
I understand and agree to the following terms and conditions of the Super Fit Membership:
- This plan is non-refundable and non-transferable.
- $25 one time sign up fee will be applied with initial purchase.
- Month-to-Month = NO CONTRACT. Requires direct monthly billing to a credit card.
*Three Month Minimum Commitment Required.
MONTHLY PAYMENT: I acknowledge and agree to authorize Modern Milk to charge the credit card/debit card on the 1st of the month billing cycle.
CANCELLATION: I may cancel my Super Fit Membership at anytime. Cancellation will take effect the next billing cycle from the cancellation date.
HOLDS: I can hold my auto-renew for up to 90 days once a year.
